RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK INDEMNITY, AND PERSONAL CONSENT AGREEMENT

I acknowledge, agree and represent, that I understand the nature of the activities, and that the same will require contact with and exposure to animals, which despite being evaluated by various personnel, are unpredictable, and that while reasonable precautions have been taken by Humane Society of Fremont County, may be hazardous. I further acknowledge, warrant, and agree that if, at any time, I believe conditions to be unsafe; I will immediately discontinue the activity in question and alert the Humane Society of Fremont County. I agree to perform my volunteer duties to the best of my ability, to always act in a professional manner, and to adhere to the volunteer guidelines set forth by the Board of Directors. I further understand that my volunteer involvement may be terminated at any time by Humane Society of Fremont County for reasons including, but not limited to those outlined in the Volunteer Guidelines.

I fully understand that being in the presences of animals involves risk and danger of serious bodily injury, including permanent disability, paralysis, and death. These risks and dangers may be due to my own actions, or inaction, the actions or inaction of others participating in the Activity, the condition in which the Activity takes place. or the negligence of the Shelter. There may be other risks and social and economic losses whether known to me or not readily foreseec1ble at this time. and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation. I will report any and all bites or scratches received while volunteering to the Volunteer Coordinator upon incident.

HEREBY RELEASE, DISCHARGE. AND COVENANT NOT TO SUE the Humane Society of Fremont County, its respective administrators, directors, agents, members, volunteers, sponsors, and other participants from all liability, claims, demands, losses, or damages on my account. I further agree that despite this release and waiver of liability, I or anyone on my behalf makes a claim I will indemnify, and hold harmless each of the parties claimed against from any litigation expenses, attorney fees, loss, liability, damage or cost which anyone may incur as the result of such claims.

I understand that I may be handling animals while providing my volunteer services for Humane Society of Fremont County and there exists a risk for personal injury. On behalf of myself, my heirs, personal representatives, and executor. I release, discharge, indemnify, and hold harmless Humane Society of Fremont County in Colorado, its agents, servants. and volunteers from any and all claims, causes of action or demands of any nature or cause connected with volunteering. This could include any costs, attorney fees, and court costs incurred by the Humane Society of Fremont County in connection with my volunteer services based on damages or injuries which I may incur while volunteering. Such damages are not limited to but may include animal bites, accidents, injuries, and personal property damage.

I agree to release, discharge, indemnify and hold the Humane Society of Fremont County harmless for any and all damage to my personal property while providing my services on a voluntary basis Lo Humane Society of Fremont County, its agents, servants, and volunteers.

I understand that public relations are an important aspect of volunteer work at the Humane Society of Fremont County. I, therefore, agree on behalf of myself, my heirs, personal representatives, and executors to allow Humane Society of Fremont County and its agents to use any photographs, videos or film taken of me for use in public relations efforts. Humane Society of Fremont County will use all reasonable efforts to notify me but notification is not required for the photographs, video, or film to be used for public relations purposes.

By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other representations made by me on this application may result in my immediate dismissal.

PARENT OR LEGAL GUARDIAN RELEASE (Volunteers 17 or younger)

As the parent or legal guardian of the above volunteer, I give my full consent to allow my child to volunteer services for the Humane Society of Fremont County and its agents as described in above volunteer contract.

I have read and fully understand the terms and conditions in this volunteer contract. On behalf of myself and my child or ward, I agree to all the terms and conditions outlined in this volunteer contract.
